Assist NDIS participants in coordinating and implementing support services based on their NDIS plan using a person-centred approach. Build strong relationships with participants, their families, NDIS coordinators, planners, and service providers to ensure effective support and service delivery.
The successful candidate will have a strong knowledge of the NDIS and NDIS funding, ability to read and interpret client plans, manage budgets and work within a KPI driven environment.
This role is permanent part time role (4 days/30.4 hours per week) which can be flexibly based anywhere in NSW or ACT with hybrid work from home arrangements.
Key responsibilities
Successful candidates will be required to clear probity checks including National Criminal History Record Check, NDIS Workers Screening Check and Working with Children Check.
